贵阳地区健康成年人血小板及其相关参数生物学参考区间的建立

摘要:

目的 通过样本数据分析,建立贵州贵阳地区健康成年人血小板计数(PLT)及平均血小板体积(MPV)、血小板比容(PCT)、血小板分布宽度(PDW)、大血小板比率(P-LCR)的生物学参考区间,为健康评估和临床诊疗提供可靠依据。方法 回顾性分析贵州省人民医院2022年1月至2023年12月健康成年体检人群,按照男女和年龄段进行分组。采集EDTA-K2抗凝静脉血,经全自动全血细胞分析仪检测,收集PLT、MPV、PCT、PDW及P-LCR数据,采用Kolmogorov-Smirnov检验数据的正态性,采用Mann-Whitney U检验比较不同性别组间差异,采用Kruskal-Wallis单程行列方差分析比较不同年龄组之间的差异。结果 纳入24 446例健康体检人员,PLT及相关参数服从近似正态分布,参考区间为男性PLT(132.79~319.57)×109/L、MPV(8.69~12.89)fL、PCT(0.16%~0.32%)、PDW(7.99~18.37)fL、P-LCR(14.72%~42.92%);女性PLT(147.59~336.15)×109/L、MPV(8.72~13.00)fL、PCT(0.18%~0.34%)、PDW(7.82~18.28)fL、P-LCR(14.51%~48.77%)。不同性别组之间、不同年龄段间差异均有统计学意义(P<0.05)。随机选取男女各100例健康体检者对新建立的参考区间进行验证,不合格率<10.00%。结论 本文建立的血小板参数参考区间符合贵州贵阳地区人群特征,经验证本参考区间可适用于临床。

Abstract:

Objective The aim of this study was to establish the biological reference intervals for platelet count(PLT) and its related parameters, including mean platelet volume(MPV), plateletcrit(PCT), platelet distribution width(PDW), and platelet-large cell ratio(P-LCR), in healthy adults in the Guiyang region of Guizhou Province, China. This study aims to provide reliable evidence for health assessment and clinical diagnosis and treatment in this population. Methods A retrospective analysis was conducted on the data of healthy adult subjects who underwent physical examinations at the People's Hospital of Guizhou Province from January 2022 to December 2023. The subjects were grouped by gender and age. Venous blood samples were collected in EDTA-K2 anticoagulant tubes and analyzed using a fully automated hematology analyzer. Data of PLT, MPV, PCT, PDW, and P-LCR were collected. The Kolmogorov-Smirnov test was used to verify the normality of the data. The Mann-Whitney U test was used to compare differences between different gender groups. The Kruskal-Wallis one-way analysis of variance(ANOVA) was applied to compare differences among different age groups. Results A total of 24 446 healthy individuals who underwent physical examinations were included in this study. Analysis showed that PLT and its related parameters including MPV, PCT, PDW, and P-LCR exhibited an approximate normal distribution. The established biological reference intervals were as follows: For the male group, PLT(132.79-319.57)×109/L, MPV(8.69-12.89)fL, PCT(0.16%-0.32%), PDW(7.99-18.37)fL, and P-LCR(14.72%-42.92%). For the female group, PLT(147.59-336.15)×109/L, MPV(8.72-13.00)fL, PCT(0.18%-0.34%), PDW(7.82-18.28)fL, and P-LCR(14.51%-48.77%). Statistical analysis revealed statistically significant differences in the above parameters between different gender groups and among different age groups(P<0.05). 100 healthy subjects of each gender were randomly selected to validate the newly established reference intervals. The non-conformity rate was found to be<10.00%, a finding that aligned with the preset validation criteria of this study. Conclusion The reference intervals for platelet parameters established in this study are consistent with the population characteristics of the Guiyang region in Guizhou Province. Moreover, validation results confirm that these intervals are applicable for clinical practice.
